• Participate in facility on-site case management activities as needed
• Complete baseline and disease specific assessments by gathering information from the member, family and treatment team

• Maintain current and accurate documentation of telephonic contacts, treatment plans, case notes, referrals, and assessments in Case Management electronic record
• Maintain appropriate documentation within protocols and guidelines of the Case Management program and the Care4U program
• Precepting and or mentoring current and new staff and serve as a resource of knowledge
• Knowledge of principles and practices of a designated therapeutic discipline
