In the previous role, I was responsible for 1:1 and group Diabetes Self-Management Education and Support (DSMES) to patients referred to the program at both the diabetes center and at several Northern Light primary care offices in the greater Portland area.

• Analytical skills necessary to evaluate, develop and implement appropriate patient education and care programs
• Board Certified in Advanced Diabetes Management (BC-ADM) or Certified Diabetes Educator in process
• Develops and provides individualized diabetes self-management/training by incorporating evidence based research, principles of adult learning, AADE self-care behavior theory to empower the patient, family with diabetes to manage the chronic disease.*
• Follows Cardiac Rehabilitation Department policies the AADE scope and standards of practice.*
• Maintains certification requirements of the program by following AADE policies and submitting annual outcomes assessment.*
• Leads diabetes advisory group meetings comprised of a multidisciplinary team that oversees quality of the program
• Documents assessment, ongoing status, interventions, patient response and outcomes in an accurate timely manner
